Abstract: Information Systems (IS) and IS project failure has been a major problem from the late 1980|s until our present day. Many organisations face this problem in varying degrees. The purpose of this paper is to identify IS and project failure indicators in order to establish a prevention model. This paper generates these indicators through a literature review and categorises them under three different headings. Finally, a predictive model to test IS and project failure is proposed.
