Diagnose the unreliability during flight takeoff

Abstract: The study is to promote flight safety level by telling the pilots to diagnose the unreliability by observing the previous accidents. Some similar airplane accidents are occurring repeatedly, and both pilots and organisations should be aware. The good skills of operating aircraft may come from the better learning from the crashes. The study method was developed as events analysis systematically on related air accidents. It showed that safer flights can be reached by education in organisations or individually by understanding the digital complexity of flying, which may go beyond the limit of the designed conditions. The symptoms were discussed about the rejected takeoff. The recognition of unreliability among hardware, software, humans and organisations is the first step to improve safety procedures. The method developed here will alert and equip the pilots with skills mentally, and interactively with computer applications in aero-technology.
