Image-Guided Adaptive Radiation Therapy with anatomic imaging and dose verification in head and neck cancer using kV CBCT

Abstract: Image-Guided Adaptive Radiation Therapy (IGART) is a promising approach in radiotherapy for bulky head-and-neck tumours. We have developed a methodology for dosimetric replanning using kV-CBCT. A case study is presented in which the CBCT and planning CT (pCT) images were registered to generate Adaptive pCT (ApCT). The registration similarity measure was intensity-based Mutual Information. The search was optimised using the 'downhill simplex' algorithm. Dose-distribution and DVH were re-calculated. The actual dose delivered can be assessed based on the ApCT. Clinical implementation of IGART has potential to compensate for inter-fractional changes, while reducing damage to critical organs and increasing tumour targeting.
