Abstract: This is an extract from a longer paper which was originally published in the British Medical Journal. Its appearance in the IJMDM arises from a conversation between Professor Wade and one of the editors, which turned on the remarkable influence of a courageous individual in the development of what might be called an ‘entrepreneurial’ initiative in an unusual (dreadful) environment. In focusing on the ‘business’ aspects of the story, the authors are well aware that these are subordinate to the human story: but there is an interplay between the human spirit and the manifestation of initiative.
